Part Time Care Coordinator

Venn Group Housing and Support Recruitment Team are currently recruiting for a permanent part time Care Coordinator, a fantastic opportunity to work with a prestigious healthcare and support provider in Bristol.
Location: Bristol
Contract Type: Temporary to permanent
Hours: 20 hours per week, Monday to Friday 9am – 5pm (Flexible working days)
Hourly Rate: £10.00 - £11.00

Day to day responsibilities include:
- Develop care plans and rearrange them as necessary if difficulties were to arise
- Maintain and develop good relations with clients and colleagues
- Visit clients to check on the care and support that they are receiving and documenting it accordingly
- Carry out assessments, spot checks and the overall domically care
- Ensure that clients health and wellbeing needs are being met and that they are receiving the correct support
- Act as the first point of contact for clients to listen to them and their needs
- Liaise with external organisations, clients and their family to develop a care package suitable for the client
- Deal with complaints, accidents, incidents and being able to document and provide written reports
- Assist with the recruitment and ongoing staff training
- Responsible for carers, overseeing their work and acting as a point of contact
- Undertake on call duties
- Ensure vulnerable clients are safeguarded
Essential skills and qualifications:
- A current enhanced DBS
- A driver with access to their own car
- A background or experience working in a care or support environment
