Venn Group have been approached by a Housing Support Organisation and would like to introduce the following position available
Position: Housing Support Worker
Location: Wakefield
Rate: £11.00 p/hour PAYE (inc hol pay) plus mileage
Length: 6 weeks with possible extension
Hours: 30 hours p/week – Mon – Fri
The role will predominantly involve:
- Caseload management of 3 – 4 clients
- Supporting patients with Mental Health needs who have been discharged from hospital and transitioning in to their own tenancy
- Working in partnership with a NHS Foundation Trust to improve adult secure services across West Yorkshire with focus on reduction in length of stay and enhancement of support within the community
- Working in partnership and based with the Specialist Forensic Community Team (SCFT), you will provide transitional support to people being discharged from secure services into the community
- Building key relationships, with focus on local housing providers and availability of appropriate housing is repeatedly identified as a barrier to discharge and will be a key focus of the team approach
- Ensuring that a robust package of support is developed and delivered which includes robust specialist forensic assessment, specialist case management and care pathway management
- Receiving and processing referrals in line with current practice guidelines
- Supporting service users of the SCFT to assess and address accommodation support issues
- Developing accommodation support plans for service users, maximising service user involvement and coproduction
- Attending CPAs and professionals’ meetings where appropriate undertaking specific pieces of housing related work
- Taking a preventative role to ensure that wherever possible, accommodation is retained, housing related hospital admissions and incidence of homelessness are kept to a minimum
The successful candidate must have worked in the housing sector and with a people with Mental Health needs to be considered, must be able to drive and have an Enhanced DBS issued within 1 year
