Venn Group are currently working on a position with a highly-regarded organisation, based in Hertfordshire, to source a Placement Officer for the following assignment:

Location: Hertfordshire (hybrid working)
Contract: 3 months+
Rate: £27.00 per hour, umbrella rate
Hours: 37.5 per week
Your duties will include:
- Accepting referrals of hospital patients or adults living in the community, from Social Services and seek to match person’s needs to available residential and nursing home resource or find alternative 24 hour provision in the community
- Providing a point of contact to those wanting information about Individual budgets then assist using relevant person centred planning tools in helping to develop holistic support plans embracing all aspects of a person’s life, and not just their immediate social care needs
- Developing and maintaining a comprehensive data base on residential, nursing homes, NHS resources, community provider agencies – both inside and outside the county to include process, facilities etc. to be able to offer information and advice to patient’s relatives, Social Services and Trust staff
- Acting as a source of information on all aspects of moving into residential, nursing and NHS accommodation, to include costs, contractual arrangements, Social Services responsibilities, benefits entitlements etc. in order to be able to offer information and advice to patients, relatives, Social Services and Trust staff
- Negotiating with the Health Authority and other Health purchasers such as GP fund holders to identify funding and help for placement of patients who require NHS continuing care funding
- Assisting with the process of taking people to choose a service provider if they are medically fit to travel
- Assisting in the identification and brokerage of community support services
- Sharing good practice and successful deliveries with local organisations, looking at innovative ways of supporting the design and development of services, which cater for the needs of the service user and encourage wider social engagement
- Carrying out appropriate administrative tasks in support of the work, maximising the use of available information technology
The successful candidate should have:
- Educating to an NVQ level 3 or equivalent, with relevant previous experience, knowledge of the Governments Personalisation Programme and legislative frameworks
- Confidence and the capability of working as part of a team and on your own initiative, with the ability to adopt and maintain a flexible working approach
- Good negotiation skills and be able to work with partner agencies to achieve common goals
- Ability to meet deadlines and always keep up to date with policies and procedures relevant to the role
- Excellent communication skills and be able to effectively communicate with a wide range of audiences in various formats including written reports
- The skills to be organised, self-motivated and confident in using ICT packages to record and report on important information
