Specialist Pharmacist
Duration: Temporary, 6 months
Location: Dorset
Hours: Monday – Friday (37.5 hours)
Rate: £21 - £22 per hour via an umbrella company
This is an interim contract for 6 months with a possible extension and you would need to start as soon as possible. This role is offering between £21 - £22 per hour (PAYE).
Outline duties below:
- Liaise with the Senior Specialist Pharmacists; Medical, Surgical, Antibiotics, Medication Risk Reduction, Medicines Information and Technical Services to facilitate the provision of a comprehensive clinical pharmacy service
- Work effectively in all teams, ensuring that decisions regarding individual patients and operational policies are multi-disciplinary decisions. Develop and build relationships to inspire confidence in the services provided
- To provide medicines information to patients and members of the multidisciplinary team, as required
- Ensuring discharge medication is provided in a prompt manner, by facilitating ward based discharge schemes and counselling patients, relatives and carers on the use and storage of medicines
- Checking final accuracy check of dispensed medicines and to identify and promptly resolve any pharmaceutical problems
- Provide clinical pharmacy services to the wards and departments liaising with other pharmacy staff to ensure that all pharmaceutical services are provided efficiently and effectively in a timely manner
- Visiting wards, working at the patient’s bedside, talking to patients and healthcare professionals. Awareness of sick and dying patients. Potentially exposed to risk of infection
- To deal with complaints where appropriate
- To contribute proactively to the prescribing process by daily ward visits while obtaining an accurate drug history from all new patients, when appropriate
- To review the clinical appropriateness of all drugs prescribed and monitor the effectiveness of all drugs prescribed
- To counsel patients, relatives and carers on the use and storage of medicines to ensure they obtain maximum benefit from their medication, where appropriate
- To ensure each patient receives their prescribed medication in a responsive and timely manner
- To ensure medicine chart rewrites are accurate
