Support Coordinator

Our client in Hampshire is currently seeking a Support Coordinator for a temporary assignment working as part of a mental health service.
Location: Southampton
Duration: 8 weeks (Initially)
Hours: 37.5 hours a week, Monday – Sunday, either (09.00-17.00 or 14.00-22.00, including sleep ins (4 week rota)
Rate: £11.06 per hour
Day to day duties include:

Managing a caseload

Carrying out risk assessments and support plans

Provide basic benefit and tenancy advice and help clients access housing benefits

Help service users to prioritise day to day tasks and weekly budget

Conduct multi agency working
-
Promote all aspects of independent living i.e. attend GP appointments and community led activities
Essential Skills:

A current enhanced DBS
