Support Coordinator
A support provider in South Somerset is seeking an experienced and highly motivated support candidate to help manage the hospital discharges of homeless patients to ensure they find accommodation, whether temporary or permanent
Location: South Somerset
Contract duration: 2 months +
Hours: 37.5 on a shift basis
Hourly rate: £12 PAYE Inclusive of holiday pay

Day to day responsibilities include:
- Actively seek to empower clients and promote independent living
- Act as the lead case worker for a specified number of clients and develop, implement and review support and safety plans, facilitating clients to lead the process
- Where appropriate, work with clients, advocates and/or representatives from other agencies to co-ordinate an integrated support/care package
- Use an asset-based approach in assessment and support planning which recognises the client’s strengths, abilities, skills and aspirations and seeks to encourage their development
- Sign-post to relevant agencies and provide support around issues such as independent living, tenancy management, welfare benefits, budgeting, life skills, employment, education, training and resettlement
- Support the clients’ to access activities and facilities of their choice within the community in line with their support and safety plan
- Make decisions in relation to referral acceptances and admittances in line with local service procedures
- Facilitate clients to access advocacy services and, where they agree, act as an advocate on their behalf in relation to issues identified in their support and safety plan
- Co-ordinate and develop lower level support / care workers where required, reviewing caseloads and practice
- Deputise for Client Services Manager / Senior Client Services Manager
- Co-ordination and lead responsibility for engagement with formal panels and processes such as MARAC, Safeguarding, team around the child/family, MAPPA, and any multi agency health meetings
Essential criteria include:
- Current enhanced DBS (within the past 12 months)
- Experience of working within supported or social housing, property management or care/criminal justice setting
- A driver with access to their own vehicle
