Assertive Outreach Worker
Venn Group Social Housing Recruitment Team are currently recruiting for a Support Worker to work for a highly reputable Support provider based in Dorset on a long term contract. The post holder will provide a valuable service to the individuals (16+) and support them in the community to become more independent and improve their overall wellbeing.
Location: Dorset
Contract Type: Permanent (£22,440 - £23,812)
Hours: Mon – Fri (37.5 hrs per week)

Day-to-Day Duties:
- Provide practical and emotional support to Rough Sleepers throughout the communities in Dorset
- Develop person-centred support plans that work with the individual to address their health, social and emotional needs in order to be able to sustain a healthy life away from the streets
- Responsible for conducting Risk and Needs assessments for the service users
- Manage and report accurate information and upload it onto to Support Plans
- Responsible for a caseload of service users (low – high) support need users’
- Work as part of a team to provide person-centred support, working with other agencies
- Work in accordance with the organisations policies and procedures to ensure Health and Safety practices are adhered to
- Attend regular staff and external meetings as requested or mandatory training modules/ events
- Report accurately any significant or untoward events to the team and Managers
- Supporting service users to access Housing Benefit, Universal Credit, and other entitlements to help them manage their accommodation
- Liaise with other external agencies, complimentary therapies and community groups on behalf of people who use our services
- Encourage service users to recognise, understand and manage factors that affect their mental wellbeing and physical health
- Maintain accurate records as required by policies and procedures, using appropriate paper and electronic systems
- Engage with community events, meetings and forums to promote the service
Essential Criteria:
- A current enhanced DBS issued within the past 12 months
Desirable Criteria:
- Experience working in Health and Social care
- A recent Enhanced DBS on the Update Service (or issued in the last 12 months)
