An exciting opportunity has arisen within a well-established Housing Association for a Housing First Coordinator to coordinate and provide a personalised, support service to people with complex needs in line with Housing First principles.

Type: Permanent

Salary: £30,117 per annum

Hours: 37.5 per week

Location: St Albans

Main responsibilities:

• Responsible for delivery of an outreach service to individuals with complex needs through individual assessment and support planning
• Provide and lead on intensive case management and wrap-around support and advocacy to service users, and coordinate and monitor referrals to community services
• Ensure service users’ individual needs are identified and met
• Facilitate and encourage engagement with service users to access services such as counselling and healthcare
• Assist service users to manage their finances, including preparing budgets
• Educate, inform and advocate for service users regarding benefits and their entitlements
• Organise and participate in social activities with service users in the community
• Assist service users to prepare for and find employment
• Facilitate educational and support groups

Essential Requirements:              

• Experience of supporting vulnerable clients with homelessness, rough sleeping, complex needs, mental health, drugs and alcohol etc.
• A full valid UK driving License and access to your own vehicle
• An Enhanced DBS with the Adults and Children’s lists checked or willingness to obtain
