Mental Health Community Navigator

Venn Group are currently recruiting for a Mental Health Community Navigator for a Mental Health charity client based in Somerset to begin a full time temporary to permanent contract to support clients who are discharged from hospital to receive specialist mental health support and integration back into the community
Location: Somerset
Contract Type: 3 Months - temporary to permanent
Hours: 37 per week Monday to Friday 9am - 5pm (Remote working and community based working)
Salary: £21,300 - £23,200

Day to Day Duties:
- Support those with various mental health conditions being discharged from the hospital into the community
- Support the individual throughout their recovery and act as a point of contact for emotional and practical advice and support
- Write and update the discharge support plan and relapse prevention plan for clients, tailored to their individual needs and progress
- Support clients to meet their daily living skills, physical, mental and wellbeing needs
- Signpost to further support services
- Identify housing options for clients and work closely with housing management staff, landlords and agencies
- Work with the CAB worker to reduce barriers to accommodation, and debt issues and advise on budgeting and how to access the correct benefits
- Maintain and update all records and attend regular team meetings
Essential Skills:
- Experience of supporting vulnerable clients with complex needs
- Knowledge of mental health and substance and misuse
- Current enhanced DBS
- Access to own car
