Outreach and Support Coordinator
Location: Yeovil
Duration: 3 Months (Likely to extend)
Hours: 37.5

Pay Rate: £11.51 PAYE
Shifts: Monday to Friday – 09:00 to 17:00
Day To Day responsibilities include:
- Assessing the needs and risks of clients and developing person-centred support plans
- Reviewing and updating support plans regularly and ensuring they are followed by the support workers and volunteers
- Co-ordinating and supervising a team of support workers and volunteers and providing them with guidance and feedback
- Liaising and collaborating with external agencies and partners to ensure the best outcomes for clients
- Communicating effectively with clients, colleagues and stakeholders
- - Upholding policies and procedures and promoting safeguarding best practices
- Co-ordination and lead responsibility for engagement with formal panels and processes such as MARAC, Safeguarding, team around the child/family, MAPPA, and any multi agency health meetings
Essential Criteria Include:
- This post is subject to a Disclosure and Barring Service (DBS) check at an enhanced level.
- Experience in developing and delivering person-centred support plans
- Knowledge of safeguarding principles and procedures for adults, children and young people
- Ability to co-ordinate and supervise a team of support workers and volunteers
- Ability to work effectively with a range of external agencies and partners
- Excellent communication, interpersonal and organisational skills
