An excellent Support provider in Somerset are recruiting for a Community Partnership Manager to ensure that people with mental health needs are discharged from hospital in a timely and safe way, and support prevent readmission through the delivery of appropriate community-based interventions.

Hours: 37
Salary: £31,257 – £34,152 (likely to increase)

Day to day responsibilities include:
- Support, lead and manage the team, ensuring effective team work, communication, and the sharing of information whilst setting a culture of hope and courage
- Ensure that services and projects run effectively on a day to day basis meeting targets as agreed with the local council and the Senior Operations Manager
- Ensure that safeguarding principles and practices are upheld and ensure that policies and procedures are adhered to at all times
- Ensure that services are delivered in a Trauma and Psychologically Informed way with co-production at the heart
- Contribute to continuous service improvement and development, through a process of learning, review and transformation
- Monitor service budget, risk and resource management within set targets/objectives, and contribute to budget setting
- Set performance targets/standards and monitor performance against these targets, with a focus on achieving outcomes and impact
- Ensure management action is taken in the event of underperformance, non-compliance, or serious incidents in order to safeguard the safety and wellbeing of service users and colleagues
- Attend MADE meetings and other senior multi-agency meetings/steering groups
Essential criteria include:
- Strong demonstrable experience of supporting vulnerable clients with various mental health conditions
- Experience in effective risk management of community based services and working within standard operating procedures
- Current enhanced DBS
- Driver with access to own car
