A reputablesupport provider in Bristol is looking for Community Outreach Worker to begin a full time temporary contract. The post holder will deliver a valuable service to the community by offering holistic and client-centred support to vulnerable individuals at risk of homelessness and with complex needs.
Location: Bristol
Contract Type: 3 Months +
Hours: Mon – Fri(9am – 5pm)
Salary: £12.66 via Umbrella

Day to Day Duties:
- Develop professional boundaries with the clients to create trust and provide support
- Liaise with professionals and housing providers; preparing clients for move-on accommodation
- Co –produce, update, and implement support plans, crisis plans, and relapse prevention tools
- Undertake defined interventions to enable the client to meet their daily living skills, physical, mental health, social care and wellbeing needs
- Identify and prevent or reduce risk to ensure risks are minimised
- Ensure the Health and Safety policies are implemented and adhered to; ensuring safeguarding issues are properly managed
- Work with a diverse range of individuals with different needs from young people to older people, LGBT service users, disabled people and with a range of backgrounds and ethnicities
- Tailor the service to meet their individual needs, challenging stigma and discrimination, advocating for person if appropriate, and ensuring up-to-date community knowledge
- Manage a caseload effectively, communicating any changes with managers and other staff contentiously
- Collaborate with other team members to maximise service performance, meet targets and Service Level Agreements, and promote effective communication and teamwork
- Maintain hope and optimism for the individual with high expectations for recovery. Use lived experience (if appropriate) to positively support the individual’s recovery
Essential Skills:
- Experience of supporting vulnerable clients with complex needs
- Current enhanced DBS
- Access to own car
