Our client is looking for a Strategic Manager – Localities & Urgent Care to carry principal strategic lead on statutory duties in relation to an area within the Adult Social Care Division, namely the Community Wellbeing Teams and Occupational Therapy team.
You will be supporting the statutory partners in developing and implementing the multi-agency safeguarding arrangements; take the strategic lead in promoting the safeguarding of children in local strategic partnerships, and co-ordinate the contribution of relevant partners in this.
Responsibilities:
- To act as Divisional expert on matters relating to Adult Social Care and the Local Health Economy pathways
- To support the Assistant Director for Adult Social Care and ensure the transformation plans and associated financial targets are understood and realised where these apply to the Hospital Team providing regular reports on progress
- To identify exceptions and risks with specific reference to the Hospital Team and put in place appropriate plans to address and mitigate risks
- To build firm relationships with our Local Health Economy colleagues including the Clinical Commissioning group leads, Midlands Partnership Foundation Trust leads, University Hospital of North Staffordshire leads and Combined Healthcare leads and National Leads in this area
- To deputise in the absence of safeguarding strategic managers and escalate safeguarding concerns to the Assistant Director / Director of Adult Social Care, Health Integration and Well Being
- To develop admission avoidance models to reduce admissions to hospital in partnership with health and voluntary sector colleagues
- To ensure residents who are on the Discharge to Assess (D2A) pathway receive safe and enablement focused care and intervention from social are at the most optimum time for the client
- To work in partnership with Commissioning colleagues ensuring effective communication between social work teams and commissioning deputising as necessary in the absence of commissioning strategic managers and respond in conjunction with the Commissioning colleagues to escalations around flow within the Local Health Economy with provision of services where appropriate
- To build excellent working relationships with Members / Councillors
- To develop excellent relationships with partner organisations to enable effective multi-agency working supports the work of the Hospital team and enhances the local offer to local residents
